Tesamorelin is a synthetic analogue of human growth-hormone-releasing hormone (GHRH 1-44), stabilised by an N-terminal trans-3-hexenoyl modification that increases resistance to enzymatic degradation. It is studied as a growth-hormone secretagogue acting through the pituitary GHRH receptor.
Binding to the GHRH receptor activates the cAMP/PKA pathway in somatotroph cells, stimulating synthesis and pulsatile release of endogenous growth hormone and, downstream, IGF-1. Unlike recombinant GH, Tesamorelin preserves the physiological feedback and pulsatility of the GH axis. Research has focused on visceral adipose tissue reduction and lipid metabolism.
